Basic requirements for receiving services at MIC:

· Must be Native American and living in Missoula County and not living on the reservation - (Arlee, Ronan, etc.)

· Must provide proper documentation upon registration (tribal ID or decadency, birth certificate, social security card, whether or not you are insured, and two proofs of residency, & registration form).

 If you are new to the Missoula Indian Center the following forms will be necessary to complete before you can obtain services from the agencies or departments
